About this Guide

Written and reviewed by the Chairlink Answers editorial team. Facts about plans and rules name a public source: the American Dental Association, ADA News, and the ADA Code of Ethics. Practical advice is marked as an office tip. The guide covers what an estimate is and isn’t, the 3 fees (full fee, contracted fee, allowed amount), starting from the right fee in and out of network, how to estimate a downgrade, checking coverage when a job changes, predeterminations, the choice to wait or go ahead, written estimates and financial agreements, what to do when the bill comes back different, and splitting care across 2 plan years. It also shows who does what: front desk, treatment coordinator, office manager, and dentist. Every story and dollar amount in it is made up, and its patient lines and scripts are examples, not real quotes. Guide 4 of 6 in the Insurance Conversations series. 19 pages, with 11 checks to run before you quote.

Tell each patient their part, in dollars, before treatment starts.

That’s Chairlink’s advice, and this guide gives your team 5 moves for the estimate talk:

1. Show: check the plan, build the estimate on the careful side from the right fee, then walk it line by line.

2. Say: give the dollar amount, and say calmly that the plan makes the final call.

3. Choose: wait for the plan’s answer, or start now.

4. Sign: a written estimate and a signed financial agreement before treatment starts.

5. Repair: what to do when the bill doesn’t match.

The careful rule: if you’re ever wrong, it should become a credit, not a new bill.

An example script for when you present the number:

“This is the most we expect you to pay: [amount]. It’s based on what your plan told us today. Your plan makes the final decision when the claim comes in. We built this on the careful side, and anything you paid that you didn’t owe comes back to you.”

And never say: “Your insurance will cover this.”

Inside the full guide: scripts for “Can’t you just call and ask them?”, “Do I have to wait for my plan?”, “You said my insurance would cover this.” and “Can we split this across 2 years?” Plus fee bars for in network and out of network, a worked downgrade example with the math check, a wait or go ahead chart, a what changed table for repair calls, 4 worked examples, and 11 checks to run before you quote.
