About this Guide

Guide 2 of 6 in the Insurance Conversations series. It covers 4 kinds of no that turn into hard talks at the desk: plan exclusions (including the missing tooth clause), downgrades (including bundles and downcodes), deep cleanings, and denials and limits. Each one gets a 1 sentence explanation, example patient lines with scripts, what to offer and what to check. 4 made up stories show the moves at work. Patient lines are examples, not real quotes, and all names and numbers in examples are made up. Plan and number lines cite public sources from the American Dental Association. Where plans, contracts or state rules vary, the guide tells you what to check. Written and reviewed by the Chairlink Answers editorial team, October 2026.

What the plan pays for and what the patient needs are 2 different questions.

When a plan says no, a patient can hear "you don't need it." This guide gives front desk teams, office managers and dentists the words to keep those 2 questions apart, and a real next step to offer.

The 4 moves

1. Listen: let them finish.

2. Name: say the plan's rule in 1 plain sentence.

3. Separate: the rule is about payment, not about what their teeth need.

4. Offer: a payment option your office has, an appeal when it's fair, or another option the doctor approves.

A preview: say it in 1 sentence

Exclusion: "Your plan doesn't cover night guards. That's a rule in your plan, not a decision about your teeth."

Downgrade: "Your plan pays for tooth colored fillings on back teeth as if they were silver fillings."
